The rapid digital transformation of healthcare has positioned perioperative medicine at the forefront of techno- logical innovation, a theme central to the inaugural ParmAI conference. This perspective synthesizes multidisci- plinary insights from the event, charting the integration of artificial intelligence (AI), telemedicine, and wearable devices across the entire surgical patient journey. While digital tools hold immense promise in shifting care from reactive to proactive paradigms across the perioperative pathway, evidence for improvement in hard clinical outcomes remains limited for several of these technologies. The central challenge for digital innovation is bridg- ing this accuracy-outcome gap. Furthermore, we emphasize that proactive governance, stringent cybersecurity, legal compliance, ethical oversight, and environmental sustainability are essential prerequisites for the safe and responsible implementation of these tools. Achieving true precision anesthesia demands rigorous interdisciplinary collaboration to ensure that AI technologies prioritize measurable patient benefits and safety.
