The recently published proposal for the 9th edition of the TNM staging system (TNM9) for HPV-related oropharyngeal squamous cell carcinoma (HPV + OPC), primarily based on imaging-detected extranodal extension (iENE), introduces significant revisions improving risk stratification [1]. However, gene expression profiling (GEP) has emerged as a robust approach to cancer classification, outperforming anatomically based staging systems [2,3]. In a previous study (BD2Decide) [4] we externally validated a 3-cluster transcriptomic model (3Cl-GEP) that stratified patients with HPVpositive OPSCC into biologically distinct prognostic groups, independently of TNM8 [3,5]. Here, we assessed the performance of this molecular model relative to TNM9 [1] within the same cohort.
