After pointing out the importance of the shoulder articular joint, the authors describe the main causes of pain. This symptomatology may have a tendinous origin and is often correlated with degenerative tears of the rotator cuffs. It has been observed that some painful shoulders improve when the cervical rachis is subjected to a treatment. In this case, pain may be caused by a segmentary trouble, C4-C5, C5-C6 by DIM. Before starting any treatment it is therefore essential to make a precise diagnosis through a careful anamnesis, an objective examination and instrumental investigations (X-rays, ultrasonography, RMN). Upon making a correct diagnosis, the therapy may be started. The same will be surgical in case of rupture of the rotator cuff, pharmaco-conservative or manipulative otherwise.
