Objective (Aims): The aim of the study is to investigate treatment outcome of mandibular advancement devices for obstructive sleep apnea (OSA).- Subjects/materials and methods: Fourteen OSA patients, 13 males and one female (53.71 +/- 8.49 years) with BMI 27.68 +/- 3.93, received a mandibular advancement splint for nocturnal use. The study protocol includes three polysomnographic recordings: before the splint delivery (for a correct diagnosis), after three and twelve months. - Essential results: Two patients dropped out complaining difficulty in wearing the appliance. Before treatment, the apnea-hypopnea index (AHI) was 24.38 +/- 11.46, the supine AHI 38.12 +/- 22.02, the lower mean nocturnal oxygen saturation 77.36 +/- 12.43. After three months of treatment, the apnea-hypopnea index (AHI) was 8.55 +/- 6.24, the supine AHI 9.07 +/- 10.58, the lower mean nocturnal oxygen saturation 85.43 +/- 4.79. Also has been calculated the mean percentile improvement: after 3 months the average AHI improvement was of the 61.8%, for the supine AHI 74%; the lower mean nocturnal oxygen saturation improved of 5%. Conclusions: all the analyzed parameters improved after three months of mandibular advancement splint treatment. It’s necessary verify the long term effects of the therapy.
Scopo del lavoro: studiare gli effetti di un apparecchio di avanzamento mandibolare nel trattamento dell’apnea ostruttiva notturna (OSA).- Materiali e metodi: Quattordici pazienti OSA, 13 maschi e una femmina (età media 53.71 +/- 8.49 anni) con 27.68 +/- 3.93 BMI, hanno ricevuto un apparecchio di avanzamento mandibolare per uso notturno. Il protocollo di studio prevede tre polisonnografie: prima della realizzazione dello splint per un corretto inquadramento diagnostico, dopo tre mesi e un anno di trattamento. - Risultati: Due pazienti hanno abbandonato lo studio lamentando un eccessivo ingombro dell’apparecchio. Prima del trattamento, l’indice di apnea/ipopnea (AHI) era 24.38 +/- 11.46, l’AHI supino 38.12 +/- 22.02, la saturazione media inferiore di ossigeno 77.36 +/- 12.43. Dopo tre mesi di trattamento, l’indice di apnea/ipopnea (AHI) era 8.55 +/- 6.24, l’AHI supino 9.07 +/- 10.58, la saturazione media inferiore di ossigeno 85.43 +/- 4.79. E’ stato calcolata l’entità di miglioramento: dopo 3 mesi la media di miglioramento dell’AHI è stata del 61.8%, dell’AHI supino del 74%, della saturazione media inferiore di ossigeno del 5%. Conclusioni: tutti I dati analizzati a tre mesi sono migliorati. Attualmente è necessaria una verifica dei parametri a lungo termine.
