Patient with keratotic nodular lesion of the right helix, about 1 cm in diameter, grown in a few months. Among the various therapeutic options we chose the surgical exeresis with histological examination. The surgical wound could not be closed with direct suture and instead of the usual free skin graft we used a flap of proximity. Among these, what seemed most suitable in relation also to a great laxity of the skin of the retroauricular furrow, was a transposition flap.
