Elderly patient suffering from senile dementia and ulcerated nodular lesion of the left eye inner canthus. Despite the non-brilliant general condition, the patient collaborated during the surgery maintaining a firm position. Once the lesion has been removed, we closed the surgical wound with a glabellar transposition flap, paying particular attention to the hemostasis and the detachment of the flap. To avoid a possible hematoma between the receiving site and the flap, a small ponytail drainage was made. In the post-operative controls, the flap is vital even if slightly raised compared to the surrounding skin.
