This study shows that topical applications of dimethyl sulfoxid (DMSO) 60% increase the benefits related to the use of intra-operating tissue expansion due to skin lesion excision. This phenomenon is explained by the strong collagen power of DMSO, which breaks both inter-molecular and intra-molecular links of collagen fibres with a possible vasodilating effect. Twenty-one patients were studied, each of them, before the operation, was treated for thirty minutes with DMSO 60% soaked gauze on the skin to expand; the skin expander and finally the serial expander (three courses of maximum hyper-expansion of five minutes each) were then applied. After removing the lesion, we proceeded to suture having previously obtained and transposed local expanded skin flaps. In terms of the volume of physiologic solution used, the results are 30% higher than traditional techniques, increasing the benefits related to the intraoperative skin expansion for the excision of skin lesions.
