The Rectus Abdominis free flap is very versatile soft tissue flap based on the deep inferior epigastric artery and vein; these vessels are long, large-diameter and ideal for microvascular anastomoses. The versatility of this donor site is due to the possibility of transferring large skin paddles with different amounts of underlying muscle to obtain the necessary thickness for the surgical gap. We have been successfully using this flap to reconstruct several defects of the neck, tongue cheek, maxilla, maxillo-orbito-ethmoidal complex, fronto-ethmoidal complex, skull base and pharyngeal wall.
